proper labeling for lotion bars?

i have tried to read through the cosmetic labeling guide and figure this out on my own first, but i have a hard time understanding unless i can see it with my eyes. like a picture or something.

so are lotion bars a cosmetic?
i have seen a bunch of lotion bars online and they just show the name of the company, the name of the scent and sometimes ingredients. it is all so confusing!

yes, lotion in any form is a cosmetic.

people are slack about the law, and they get away with it. *I* do MY best to comply, but I seem to be in a minority.
 
my mom is hostessing a jewelry party and wants to give away some of the things i make for myself as gifts there. i know how to label my soap but i want to be 100% positive i am labeling the lotion bars correctly.

so i could type my name, the scent and purpose of the product like Lavender Lotion Bar, and the amount on a sticker and that is all right?.... do i need to add anything else to make it meet the requirements of the law?
 
so you put where you live on your labels? we are super paranoid about info like that getting into the wrong hands (dh worked news for waaaaaay to long, to many bad things he saw)
do i *have* to add my address or is that part optional?
 
the law is that you need to provide a physical address (not just a PO Box). but I know many crafters do not do so.
 
ok, i guess i will add my address then. i want to be legal after all...

They need your address so they can find you for a number of reasons. If someone had a reaction they would want to be able to contact you for more info, etc.
