honey separated out of soap

I recently made an oatmeal soap with two tablespoons of honey. When I checked the mold (I use a six seater) the honey had separated out into one bar and pretty much gone really granular and messy. Any ideas on what I did wrong there?
 
I know not everyone does it this way, but this is how I was taught to add honey when making soap. I stir the honey into the lye solution while it's warm, not hot. This completely dissolves the honey and I've never had a problem with separation.

Honey can be a tricky additive to use in soap depending on how it's added. I used to get 'weeping honey' in my honey soaps and also 'honey spots' all the time until I changed the way I added it. In this post here I describe how I add my honey. Since doing it this way, I no longer get weeping honey or honey spots in my soap.

How did you add your honey?

Edited to add- just saw Navigator's post. We pretty much add our honey the same way, only my lye solution is room temp when I add the honey to it.

I stirred the honey into the lye/oils mixture at trace - is that wrong?
Others may disagree, but I use 1 tablespoon honey PPO. I warm a honey/water 50/50 mix in the microwave and then add it to my warm oils, SB for a full minute, before adding the lye solution. Years ago, I added it after the cook when making HP Honey & Oat castile. It kinda sizzles when I do that, but it mixes right in. I've never had a problem with it and I make a lot of honey soap for my wholesale customer around the holidays.
 
I am so so so sorry but I don't know what some of those terms mean (I'm *really* new) what is PPO and SB and HP? I really appreciate your help so much!!!!!
