I was thinking of making a form on my website where people can design their own soap such as choose a base recipe, scents, colors, additives and shapes.

I would clearly state the soaps need to cure for X weeks before being shipped depending on the base recipe they choose and would have a minimum order quantity to make sure it's worth it (I don't want to make a custom batch for one bar).

Does anyone else do this and do you get a lot of custom soap requests?

If I do this would it be OK to request payment in advance to avoid making a custom batch for someone for them to then say "I changed my mind, I don't want them anymore" once they're cured?

def ask for full payment up front, or at least 80%. no money, no soap, simple. these days, i don't believe promises made thru words. you want my product? you pay, and i'll ship it.
 
Yes, payment up front, just like any other purchase. And a minimum order, for a custom loaf. Maybe the option to save their recipe for future re-orders. Might be a good idea to have only a couple choices of base, normal and extra conditioning maybe? And have those premade, so you can just melt and rebatch with their custom add-ins, and not make them wait so long for curing.
